​On July 4, 2025, President Donald Trump signed the One Big Beautiful Bill Act into law. The tax and spending cuts bill made substantial changes to Medicaid, but the American Health Care Association (AHCA) and its member advocates were able to shield nursing homes and intermediate care facilities from any reductions to provider taxes. Another major victory for providers is the 10-year delay of the federal staffing mandate for nursing homes, essentially a repeal by legislative standards. AHCA urged lawmakers to address this unworkable rule, and its inclusion in the final package reflects the success of those efforts. 

These positive outcomes reflect lawmakers’ growing recognition of the need to protect seniors and individuals with disabilities in long term care.

Member engagement in advocacy played a vital role. In June, more than 750 AHCA and National Center for Assisted Living (NCAL) members participated in Congressional Briefing, completing over 250 Capitol Hill meetings. From January to May, members visited more than 150 offices in nearly 30 states, including over 65 meetings with key committee members.

Efforts to protect Medicaid remain ongoing, but AHCA/NCAL and its members have shown the power of unified advocacy to protect and support the long term care profession.​
